Birth Certificate
Original and one photocopy for verification.
Passport Photos
Two recent passport-size photographs of the child.
Health & Immunisation Record
Up-to-date clinic card or immunisation booklet.
Parent/Guardian ID
A copy of the parent or guardian's national ID or passport.
